Welcome to CEREVITY, Licensed Clinical Social Worker, P.C. (“CEREVITY,” “we,” “us,” or “our”). These Terms of Service (“Terms”) govern your access to and use of our website https://cerevity.com (the “Site”) and all services we provide, including virtual mental health therapy (the “Services”).

By using our Site or Services, you agree to be bound by these Terms and our Privacy Policy. If you do not agree, please do not access or use our Services.


2. Eligibility

You must be at least 18 years old to use our Services. By using our Site or Services, you confirm that you meet this requirement and are legally capable of entering into a binding agreement.


3. Our Services

CEREVITY offers virtual mental health services delivered by licensed clinical social workers and other qualified professionals. Services are currently available only to individuals physically located in the state of California at the time of service.

Our Services are not intended for crisis situations. If you are experiencing an emergency, call 911 or go to your nearest emergency room.


4. User Accounts

You may be required to create an account to access certain features. You agree to:

  • Provide accurate and up-to-date information

  • Maintain the confidentiality of your account credentials

  • Notify us immediately of any unauthorized use of your account

You are solely responsible for all activities that occur under your account.


5. Payments and Refunds

Some Services may require payment. You agree to:

  • Pay all applicable fees at the time of service

  • Provide accurate billing information

  • Understand that payments are processed via third-party platforms

Refund Policy:
Fees are generally non-refundable, except as required by law or under exceptional circumstances, evaluated case-by-case. Please contact us if you believe you are entitled to a refund.

To the fullest extent permitted by law, CEREVITY shall not be liable for any indirect, incidental, or consequential damages arising out of your use of our Services. Our total liability will not exceed the amount paid by you in the twelve (12) months preceding any claim.
