The question every private client asks first

Where does this end up, and who could ever see it?

When your name is on foundations, filings, and family offices, the concern is not embarrassment. It is exposure. Here is exactly what private-pay care never creates.

No insurance record exists

No claim is filed, no diagnosis code is generated, and no carrier database ever records that you attended. There is nothing in an insurance system for an underwriter, a journalist, or opposing counsel to find, because it was never created.

Nothing enters the family office

No invoice needs to pass through a family office, an assistant, or a bookkeeper unless you choose it. Your clinical record is held by your licensed clinician alone, under HIPAA and legal privilege.

No waiting room, no sightings

Telehealth-only by design. There is no lobby, no parking lot, and no chance of being seen entering a building. Sessions happen from your study, your boat, or wherever you actually are that week.

What actually walks into session with a private client

Not generic stress. Six patterns our clinicians treat every week at this level of wealth.

01The suspicion of every relationship

The permanent question of what each person actually wants from you, and the exhausting arithmetic of never quite knowing.

02Guilt without a grievance

You are not allowed to be unhappy. Everyone says so, including you, and the feeling does not comply.

03Purpose without necessity

Nothing has to be done by you, ever again. It sounds like freedom until the morning it is not, and there is no vocabulary for that among your friends.

04The inheritance that arrived with a script

A name, an obligation, and a life plan you did not write, plus the private knowledge that you cannot decline any of it without becoming the villain.

05The next generation

Raising children inside a level of comfort that removes the forces that built you, and watching what that does, without any idea how to intervene.

06The liquidity event afterlife

The wire cleared, the identity did not follow, and the emptiness afterward is a thing nobody warned you about and nobody will sympathize with.

What the work actually looks like

Clinical treatment, delivered at the standard of discretion you already expect from everyone else you retain.

The first month

The opening sessions establish what is actually wrong, which at this level is rarely what the outside world would guess. Whether the flatness is depression or the absence of necessity. Whether the anxiety is clinical or the accurate perception of being surrounded by people with an interest. Validated instruments give a baseline rather than an impression.

By session three or four there is an explicit formulation and an approach chosen for it. You will be told what the plan is and how it will be evaluated, because you have spent your life retaining people and you know the difference between a professional and a flatterer.

How it fits a private life

Wealth changes the room. A clinician who has not worked with private clients tends either to be intimidated, and therefore useless, or subtly punitive, and therefore worse. What you need is someone entirely unimpressed, who treats the money as a set of forces acting on a person rather than as the point.

Structure matters here for a specific reason: when nothing in your life is externally required, an unstructured hour can become one more thing that drifts. A focused session with a plan and measurable progress is what makes this different from an expensive habit.

What tends to change

Early, usually sleep and the low-grade dread, and the sharpness at home. Then the isolation moves, because one relationship now exists in which nobody wants anything from you and nothing you say can be used later. That is rarer in your life than it should be, and it is doing more damage than you think.

The longer work is about purpose and about the family: constructing a life that is chosen rather than inherited or defaulted into, and changing the way the next generation experiences you before the pattern sets.

Therapy, not coaching: the distinction matters here

Much of what private clients find when they search for help is executive coaching. It has value for skill-building, but it cannot diagnose, treat, or legally protect what you disclose.

Where we practice

Nationwide

Coverage across the United States: our psychologists hold PsyPact authority spanning the participating states, and individually licensed clinicians cover the rest, including states outside the compact. You tell us where you are; matching handles the licensure.

No office. On purpose. No commute, no waiting room, no chance encounter with someone from your board, your OR, or your firm.

One seat, one story

I sold the business at forty-six and spent two years being told how lucky I was, which is true, and being unable to get out of bed some mornings, which was also true. You cannot say the second part to anyone. Not to friends, not to family, certainly not to people who work for you. The value of an hour with someone who has no stake in my life at all is very hard to explain until you have had it.

You have advisors for the estate, the taxes, the art, and the plane. There is one seat still empty.

Questions private clients ask before starting

How is discretion actually protected here?
Structurally rather than by promise. Private-pay means no insurance claim, no diagnosis code, and no carrier record is ever created. Telehealth means no building, no lobby, and no arrival to be observed. Billing can be arranged so that nothing routes through a family office or an assistant. Your clinical record is held by your clinician alone under HIPAA and legal privilege.
I am abroad or between residences for much of the year. Does that work?
Sessions are held seven days a week, 8 a.m. to 8 p.m. Pacific, and your clinician plans around a calendar that moves. Licensure follows where you are physically located during a session, so tell your intake coordinator the real pattern of your year and we match you with clinicians licensed for the places you actually are, rather than discovering the constraint later.
My family is the problem, not me. Is this still the right thing?
Often the individual work is where it starts, because the family system is easier to change from a stable position inside it than from the middle of the conflict. Your clinician can also work with the family or coordinate a family session where that is clinically indicated. What we will not do is take sides on the money.
Is this therapy or is it wealth counseling?
It is licensed clinical therapy. We are not financial advisors, we do not manage assets, and we have no interest in your allocation. What the wealth does is shape the psychology, and clinicians who have never sat with a private client will often miss it entirely or, worse, be quietly dazzled by it. That is the specific competence being matched for.
How much does private-pay therapy cost?
Session fees are published on our pricing page. Most PPO plans reimburse 60–80% of out-of-network session costs after deductible, if you choose to file. Many of our clients deliberately don't, keeping care entirely off insurance records.
Why does private-pay matter for someone in my position?
Insurance billing creates a diagnosis code that is stored and shared with your carrier, and it can surface in life-insurance underwriting, licensing reviews, clearance investigations, and legal proceedings. Private-pay means no code, no claim, no third-party record. What you say in session stays in session.
