Reserved Clinical Availability for One Client or Family
A psychologist on retainer, for people who want one call, not a search
A therapist on retainer is not a bundle of prepaid sessions. It is a standing arrangement in which one licensed clinician holds availability for you, answers first, and already knows the situation before you have to explain it again. Same-day priority. One dedicated psychologist. 100% private-pay. Nationwide.
The short answer
A therapist on retainer is a licensed clinician who reserves availability for one client or family under a standing arrangement instead of booking session by session. At CEREVITY that means one dedicated doctorate-level psychologist, same-day priority for scheduling, and nationwide telehealth. Care is private-pay, so no claim, diagnosis code, or insurance record is ever created.
The honest part, before anything is signed
What a retainer is, and the two things it is not
Most retainer pages imply everything and commit to nothing, which is why they are hard to trust. Here is the shape of the arrangement in plain terms, limits included, because the limits are what make the rest of it real.
Reserved time, not unlimited access
A retainer holds a defined block of your clinician's week and moves you to the front when something needs an hour today. It does not mean an unlimited session count or a clinician on call at any hour. What you are buying is priority and continuity, written down before you start.
Not crisis coverage, and we will say so first
No private arrangement is crisis coverage, and anything sold that way is describing staffing it does not have. CEREVITY is not a crisis service. When safety is the question in the moment, 988 and emergency services exist for exactly that, and your clinician builds a plan with you for that hour instead of pretending it will never arrive.
No claim, no code, no carrier file
Because the arrangement is paid privately, nothing is submitted to an insurer: no claim, no diagnosis code, no third-party record. The only clinical record sits with your licensed clinician under HIPAA and legal privilege, released solely on your written authorization outside the narrow legal exceptions explained at intake.
Who actually keeps a psychologist on retainer
Less a type of person than a type of season. Six situations that send people looking for a standing arrangement instead of a waitlist.
Founders in the middle of a raise
The diligence window runs eight weeks and every one of them is load bearing. You cannot start a clinician search in that stretch, and you cannot afford to hear that the next opening is in March.
Executives in transition
A merger, a succession, an exit, a board that changed its mind. The pressure arrives in bursts nobody schedules, so the hour has to exist in the week it is needed rather than the month after.
Families served by a family office
The office already retains counsel, a CPA, and a physician. Mental health is usually the empty seat, and filling it privately keeps it inside the same circle of confidence as everything else.
People who have already run the search
You called seven names off a directory, left five voicemails, and heard back from one clinician who was not taking new clients. A retainer exists so that call happens once, at the start, and never again.
Roles with no margin for a waiting room
Public figures, principals of visible firms, people whose calendar is run by someone else. Discretion is not a preference in these seats; it is the reason the arrangement is built this way at all.
Couples and parents who want one clinician who knows the file
Repeating eighteen months of history to a new intake coordinator is its own kind of cost. Continuity with one clinician means the work resumes where it stopped rather than starting over.
What the retainer includes, and what it does not
Specifics, because a retainer described only in adjectives is not a retainer. These are the terms that go in writing.
What is reserved
Your clinician holds a recurring slot in the week, plus a defined amount of held availability beyond it: the hours you can pull forward when something breaks on a Tuesday. Sessions run seven days a week, 7 AM to 9 PM Pacific, so the reserved hour can sit before the first meeting of the day or after the last.
Retainer clients receive same-day priority for scheduling and rescheduling, which in practice means your request is placed ahead of the general queue rather than into it. Brief coordination by secure message between sessions is included; that channel is for logistics and short check-ins, not a second therapy hour.
What is not included
A retainer does not include unlimited sessions, after-hours availability, or crisis and emergency response. It does not include medication management, which belongs with a prescriber, and it does not include letters, evaluations, or opinions written for courts, employers, boards, or underwriters.
It also creates no insurance footprint, because none is generated at any point: no claim is filed, no diagnosis code is assigned, and CEREVITY does not issue superbills. If a document is ever needed by a third party, that is a separate conversation with your clinician on your written authorization, never something the arrangement produces quietly in the background.
How it is scoped, priced, and ended
Scope is set in a first conversation: who the arrangement covers (one person, a couple, or several members of a household), how many hours are held each month, and how the reserved time is meant to be used. That scope is written down and reviewed on a set cadence instead of drifting quarter by quarter.
What it costs follows the scope: how many hours are held, and for how many people, quoted to you in writing before anything begins. Current session rates are published on our pricing page. A retainer is also not a lock-in. It ends on notice, and clients who no longer need reserved availability continue session by session with the same clinician.
Retainer, membership, or paying session by session
Three ways to buy the same clinical hour. A retainer holds flexible availability for you, a concierge membership holds a fixed standing slot each week, and paying session by session holds nothing beyond the hour already on the calendar.
| Psychologist on Retainer | Session by Session | |
|---|---|---|
| What you are actually buying | Reserved availability and continuity with one named clinician, scoped and priced monthly | One hour at a time, booked whenever a slot happens to exist |
| When something breaks on a Tuesday | Same-day priority: your request goes ahead of the general queue | You take the next opening, which may be next week or next month |
| Continuity of clinician | One clinician who already holds the history, so the work resumes rather than restarts | Continuity lasts only as long as their calendar stays open to you |
| Closest CEREVITY alternative | A concierge membership, which reserves one fixed weekly slot instead of flexible held hours | Nothing to compare; you are simply booking an hour |
| Choose it when | The calendar is volatile, the stakes are high, and waiting two weeks for an hour is the actual risk | The week is predictable and a standing hour is all the access you need |
Concierge by design: you never browse a directory
One conversation about what has to be held. One deliberate match.
Where we practice: nationwide. Coverage is nationwide: our psychologists hold PsyPact authority across the participating member states and individually licensed clinicians cover the rest, always matched to where you are physically located during the session. Nothing about the arrangement depends on a city, and there is no office for anyone to watch you enter.

Reserved hours still have to be a length. Most retainer clients build around a weekly standard hour and keep the longer formats available for the weeks that need them.

One founder, mid-raise, on what reserved time changed
“We were in the middle of the raise when a bad week hit with no warning. Normally that would have meant either waiting three weeks for an opening or trying to handle it myself while everything else was on fire. Because the time was already reserved, I didn’t have to choose. I used the hour, went back into the meetings, and the rest of the week didn’t spiral the way it usually does when something lands sideways.

The point of a retainer is that the hardest week is not also the week you start looking.
